The health assessment provides a snapshot of your current health and health risks as well as tips on practical things you can do to improve your well-being.
Completing the online health assessment is quick and convenient. It takes just 15 minutes, and you can take it on your computer or mobile device.
The health assessment asks you a variety of basic, easy-to-answer questions about your diet, exercise, sleep, medical history and lifestyle factors. You’ll immediately receive a health score based on an analysis of your modifiable health risks, plus tailored reports specific to your health risks. Knowing your risks can help you take charge of your health and make lifestyle changes to improve your well-being, now and in the future. The U.S. Well-Being program gives you tools and resources to improve your diet, reduce stress, boost financial fitness and much more.
Upon completing your assessment, you may receive an outreach call from a WebMD Health Coach. Everyone can benefit from a coach (even top professional athletes have health coaches), even if just to help you set some goals. Plus you’ll receive 20 points for each coaching session and a $75 gift card after completing just 4 coaching sessions by August 30, 2024.
How to complete the health assessment
After you register on the U.S. Well-Being platform through WebMD, you can continue to log in online or use the Wellness at Your Side app. Once logged in, click on the Health Assessment card under your “Take Action” section or from the menu in the upper right of the screen, select Health Assessment.
If you can provide your current biometric screening numbers when you complete the health assessment, your results will be more accurate. But you can complete the assessment without them.
Points for completing the health assessment
If you complete your health assessment by the “early bird” deadline of March 31, 2024, you’ll receive 20 points plus 10 additional “early bird” points for a total of 30 points.
If you complete it from April 1 – August 30, 2024, you’ll earn 20 points.
